New Patients at ChiroSource Madison
When you book your appointment with ChiroSource Madison, we’ll walk you through the process and tell you what to expect. We’ll send you a link so you may complete your new patient paperwork online before arriving. If you’d rather complete the forms in our office, simply come in a few minutes early, and we’ll help with the paperwork.
Your First Visit
Our friendly team will greet you by name and take copies of your ID and insurance card to verify your benefits. We’ll take X-rays and show you around the office, and then you’ll sit down with your chiropractor for an in-depth consultation. He’ll talk about your health history and what has prompted you to seek our care.
Your Second Visit
At this appointment, your chiropractor will review his findings report and walk you through everything: what’s wrong, what we can and can’t correct, and how much everything costs. If you’re ready to begin, we’ll start your care plan at this visit.
Your care plan recommendations may include adjunctive services such as spinal decompression, manual therapies, heat, stim, sports rehab or functional movement exercises. Every patient’s care plan is customized to their needs and goals.
